Stride Services Privacy Notice
Section 1: About Stride Services
Stride Services, part of the Institute for Disability Research, Policy & Practice at Utah State University, offers community-based supports for youth and adults with developmental disabilities, including inclusion, integration, individualized supports, and employment related activities. To provide these services and promote safe participation, Stride Services collects the personal information needed to understand and support each participant.
Section 2: What Information We Collect
Stride Services may collect a variety of personal information to support your participation, including:
- Contact and demographic information
- Guardian and emergency contact information
- Disability‑related information relevant to support needs
- Communication preferences or accommodation needs
- Daily support information, activity participation, and progress
- Information needed to coordinate with support coordinators
- Information required for administrative reporting (e.g., service dates, service codes, attendance, and billing units)
- Limited health‑related information only when needed for safety or required by state agencies
- Media/photography consent preferences
We collect information directly from you, your guardian (if applicable), or your support coordinator.
Section 3: Why We Collect This Information
We use personal information to:
- Provide individualized supports, day services, and community/employment‑related activities
- Develop and carry out individualized support plans
- Promote safety during participation
- Communicate with you or your guardian
- Coordinate with state support systems, support coordinators, and authorized partners
- Meet required state reporting and administrative obligations
- Maintain program records as required by Utah State University
- Support program operations, quality, and compliance with funder or regulatory requirements
Stride Services does not provide medical care and does not use your information for healthcare treatment or billing.
Section 4: When We Share This Information
We may share information with:
- Stride Services staff providing or supervising your supports
- Support coordinators and authorized state agencies when required
- Emergency responders if needed for health or safety
- Other individuals you authorize
We do not share information for marketing or commercial use.
Section 5: Data Security & Retention
Stride Services uses administrative, technical, and physical safeguards required by Utah State University and applicable law. Access to your information is limited to staff who need it to support your participation. Your data is retained for as long as necessary to fulfill the purposes described in this consent form and as required by university policy and applicable law.
Section 6: Your Privacy & Questions
You have the right to:
- Access your personal information
- Request corrections to inaccurate information
- Request a copy of the data Stride Services maintains about you
- Ask questions or file a complaint about how your information is handled
