STRIDE SERVICES GRIEVANCE PROCEDURE

Stride Services has developed a grievance procedure to help assist with a problem or concern in a timely manner. It is your right to let someone know if you are unhappy or if you would like something changed.  All issues will be discussed in a confidential manner. 

If you are unhappy or if you would like to change something, here is what you do:

  1. First, you should talk to or write a note to your staff. They can be reached at Stride Services during the day at 435-797-8532.
  2. If your staff is unable to help you, or you would prefer to talk to the director, please contact Drake Rasmussen. He can be reached at 435-797-8528.
  3. If the director is not able to help you, you can contact your DSPD Support Coordinator.
  4. If you are not satisfied with the solution that is agreed upon by Stride Services and your DSPD Support Coordinator, you may contact an outside advocate or legal service of your choice.
