How to get started with Early Intervention:

Up to 3 is the EI program for Box Elder, Cache and Rich counties. If you are looking for South East Early Intervention, visit their website.

Referral (Free)

If you have concerns about your child’s development, you, your doctor, daycare, or others can request a free evaluation from Up to 3 or your local EI program. You’ll be asked to complete an Ages and Stages Questionnaire (ASQ) online, or you can call Up to 3 at (435) 797-3727 to get more info before getting started.

An Up to 3 provider will then call you for an intake interview within a few weeks.

Intake Interview (FREE)

 An Up to 3 provider will call you to help you:

  • Learn about EI and the evaluation process.
  • Schedule a health, hearing, and vision screening with Up to 3 nurse (this usually takes place via video call).
  • Schedule your initial evaluation and any other necessary developmental assessments.

Evaluation (Free)
Your child will need to be assessed to measure developmental progress in motor skills, cognitive abilities, and social-emotional skills before beginning services.

About the assessment:

  • It takes place at your nearest Up to 3 testing site.
  • It takes about 2 hours.
  • Evaluators will observe and engage your child in tasks.
  • Evaluators will ask you about your child's development.

If other assessments are needed, these are generally conducted at your home at a later date.

If eligible, you’ll work with a service coordinator to create an Individualized Family Service Plan (IFSP) outlining goals for the next six months.

The IFSP will include:

  • Services you will receive, where and how often.
  • Resources, priorities and concerns.
  • Expected outcomes.
  • Info for transitioning out of EI.

Services (Medicaid/CHIP or Sliding Scale)
Services generally take place at your home, 1-2 times a month. You will be coached on daily strategies that will help you work with your child throughout the month. Other services can include classes with other parents and children.

EI is a team effort. Providers are here to help you with your concerns and priorities.

Exiting Early Intervention
If you are not eligible, or your child has graduated or aged out of EI, your service coordinator will share resources to best help your child move forward.

Eligibility will be evaluated annually. An exit evaluation will occur to prepare for a transition out of EI.
