Wanted: Your Opinion

Dear Utahns with disabilities, families and service providers:

This is your chance to influence our disability priorities in Utah—and also the work we do with our partners around the state.

The USU Institute for Disability Research, Policy & Practice is conducting a statewide needs assessment survey to evaluate what is working well and what is not working well for people with disabilities in Utah. We would like to invite you to complete this short survey to help us better understand the issues that are of concern to you. It should take about ten minutes to complete.

Your responses in this survey are completely anonymous and cannot be connected to your identity in any way. The information you share with us will be used internally to help us improve our programs and services. Your input will help guide our program development and planning efforts over the next few years. This survey should take approximately 10 minutes to complete.
