Enhancing Home-Based Care and Transitional Support for Individuals with Disabilities in Utah
Project Abstract
This project, led by the Homecare and Hospice Association of Utah (HHAU), aims to address critical gaps in caregiving education for individuals with disabilities across the state. This three-year initiative will deliver specialized training programs focusing on mental health support, family caregiving, dementia care, and care for incarcerated individuals with disabilities.
To ensure accessibility in underserved geographic areas, HHAU will utilize a multichannel approach, including online modules, live virtual sessions, and in-person events across Utah’s rural and urban communities. Partnerships with key stakeholders and community partners, such as the Huntsman Mental Health Institute and Utah’s home health, hospice, and personal care providers, will facilitate outreach and
implementation.
A core element of this initiative is its commitment to accessibility. Training materials and delivery methods will adhere to the latest Americans with Disabilities Act (ADA)
guidelines by incorporating screen-reader compatibility and alternative formats,
including printed and audio/visual resources. In-person events will be hosted at ADAcompliant venues. Outreach efforts will leverage community networks, trade
associations, and digital platforms to ensure broad and equitable access.
As Utah’s leading advocate for home-based healthcare, HHAU is uniquely positioned to implement this initiative due to our established relationships and extensive experience in developing caregiver training programs. By equipping 10,000 professional and/or family caregivers with specialized knowledge and resources, this initiative will enhance the quality of home and community-based care, empowering individuals with disabilities to live with dignity and support and facilitating their transition between care settings.
